Congregate Meals
We serve a hot nutritious meal at noon for those who want to get out and socialize, play cards, bingo and enjoy some great entertainment. It’s also a great place for seniors who are alone and just don’t fix a good meal for themselves anymore. You don’t have to be a senior to come and eat at the center. We welcome everyone to our center.
Home Delivery Meals
This program is for older individuals who are homebound and unable to leave their home without assistance because of a disabling physical, emotional, or environmental condition, and are unable to independently prepare a nutritious meal. We can deliver a hot nutritious meal to their home Monday through Thursday and frozen for those who need meals to get them through the weekend.
Please call the center if you are in need of meals. We can start you immediately if you qualify. Eligibility is determined by filling out an approved client registration form. If the client needs meals in excess of six weeks, an AAA (Area Agency on Aging) case manager will visit and they will determine individual eligibility. You will be re-evaluated on an annual basis after the initial visit.
The suggested meal donation is $3.00 per meal.
We are also authorized to deliver Medicaid waiver meals to the aged and disabled who meet the Medicaid waiver program criteria through their case worker.
Transportation
Our van service is available to those
who need transportation from within the area of Southern Fremont County, which consists of St. Anthony, Chester, Parker, Teton, Wilford and the Egin area, to the South Fremont Senior Citizen's Center on Monday and Wednesdays for congregate meals, and to the
Ashton Senior Citizen's Center on Friday for congregate meals.
Our van service is also available for transportation to medical appointments (within the community), grocery shopping, pharmacy, post office, hairdresser and bank between 10:00 a.m. and 2:00 p.m. on Monday's and Wednesdays. Please make arrangements
with the driver in advance.
Our van service also travels to Rexburg for transportation to medical appointments, grocery shopping, pharmacy, post office, hairdresser and bank between 10:00 a.m. and 2:30 p.m. on the 2nd Tuesday of each month. There needs to be at least three scheduled people or we can't make the trip. Please make arrangements
with the driver in advance.
We also have another van that transports "Foster Grandparents", a St. Anthony Juvenile Correction Center (JCC) program, to and from the JCC facility, between 8:00 a.m. and 2:00 p.m., Monday through Friday.
When weather & schedule
permit, the Friday van will take passengers on a ride around the countryside
after lunch. |
